I have several forms I use for access requests, break fix issue reporting etc.
I was wondering If there is a way to make the form dynamic so that if / then statements can populate new fields based on the previous fields response.
Example:
In a select list custom field (cascading or multiple choice) if user selects a particular variable, a new field will appear below it.
User selects department engineering > field below will list all relevant applications for the engineering department

Atlassian has recently rollout "Forms" feature that includes what you are looking for. It works different than the normal request forms but you can include this form in the Portal.
You should be able to implement logic to show/hide fields by using Sections

Thanks for this answer. Not sure how recent this was released but it doesn't seem to be available on my site. When I go to project settings, I do not see a Form section as described here:
From the navigation settings on the left, select project settings > forms

has anyone noticed when using the user picker fields there is a spinning circle like its trying to load or refresh after the form submission
