How to project particular filed (Secondary)while clicking on some filed in jira customer portal form

While clicking on some filed. eg : Yes or no. if we are click on Yes some addidtional field need to came up in below of this field.( For eg: Required backup. fields Yes/No. If we are click on Yes the application name need to project with radio button. If it is No, no extra fields required)

This can be done with the built in ProForma forms in Jira Service Management projects. Use the Sections to accomplish this. The following article should help guide you.

The use case you described can be accomplished with Dynamic forms which, as a part of Extension, allows you to turn the request forms to act dynamically and configure fields that are dynamically displayed based on the above selection. This should help you break the request form into more levels by defining relations between fields. 🙂
