How do I make a custom field appear on an issue creation screen?

My example: we have a form that a person fills out for unblocking a website via our filtering, which the user gets to by selecting a link on a previous Jira page. I cannot figure out how to make a custom field (a static piece of text with instructions) to appear on the form at the top. We are using Atlassian Jira Project Management Software v9.6.0. It might just be that I haven't fully grasped the fundamental concepts of where objects exist and how they are configured, but I am hoping someone can help with the limited information I can provide.

Hello @Daniel Good and welcome to the Community!

 

I noticed that you tagged Jira Service Management. If so, then you can include special instructions for filling out the form by going to the Request Type settings and adding text in the Instructions box. This information would appear for your end users using the customer portal. 

More information is available here.

Below is a screenshot of where you can include this kind of info.

Awesome, thanks! I was focusing too much on the global settings, not project settings and once I selected request types, it all became clear. :D
