How can I set up a task request form?

I need to set up a task request form that can be sent via link to customers and employees to fill out when they need a deliverable from the Marketing department. 

Once the form is submitted, I want it to show up as a new task on the kanban board so marketing knows they have to begin working on it. 

Is this possible?
