Currently the Story Points field is only showing when I am on the Issue View screen, but I need to add it to the Request Form screen so I can assign it a default value. However, when I search for the field, it is not available to add.
In your image there is the Story point estimate. Is that not what you want? I believe it is.

So I tried the story points estimate field but it's not the same as story points. I'm still looking to try and figure out how to add the field called Story Points to the Request Form screen when creating a new request type.

Ah yes. I think the Story Point Estimate is from one of your TMP projects. You need to ensure that SP is configured for your JSM project context. Interestingly here is what I see on one of my JSM projects. In any event ensure that as an admin you see the SP field when you attempt to create an issue w/o a request type selected using the "+" button. If you do not the you need to configure the field in the scope of your project. Some guidance on this is provide when you attempt to create the issue and select the ellipses and Find your field.
