I have recreated the forms under JSM Forms. But the problem is that the PDF output format the submitted form provide doesn't look good. Is there anyway i can change the Output Format.

Thank you.

Hi @sohel , if I understand your question correctly, you can't do much with the formatting of a JSM form with out-of-the-box capabilities. However, there might be a third-party app in the Atlassian Marketplace that would help you do that.

One other thought (although I haven't done this) is, if you also have Confluence on your instance, you might be able to create the form in that (which does provide formatting capabilities) and link it to JSM issues.

Hi @sohel , Welcome to the Community, we're glad you're here!

JSM doesn't have the ability to import Word documents as forms, but you do have options:

  1. You can create an editable PDF file from the Word form and attach it to the issue with a Comment to the user to complete it and attach it to the request/issue.
  2. You can recreate the form in JSM under Project settings > Request Management > Forms and the new user can complete it. If you recreate the form in JSM, the data will actually be part of the request/issue, and then you can link the form fields to Jira fields so that the field data can be used for filters, reports, queries, etc.

I hope one of these options suits your needs!
