I made a request type that has an associated custom form with several text and checkbox fields. This form works in the Portal just fine and I confirmed that a user does not need to login to access the portal and complete the form.
But when I select the same request type from my widget, the only field that comes up is an email address. The rest of the form does not appear.
Any thoughts on why this is happening? How to fix?
Is there an answer for this yet? Forms still are not showing on the widget. Any workarounds?
Any solution to this? Forms are much more flexible to use than individual request fields.

What do you mean by "my widget"? Can you provide more context about it?

Here's my widget that is inserted in the page. The form I am having issues with is the "Request a new Caseworker Account". This form works on my portal (https://fccc-technology.atlassian.net/servicedesk/customer/portal/5/group/-1) but does not work on the widget. On the widget, the only field is "Your contact email".

Good question as I hadn't thought to check as this is all new! No, none of the forms are working on the Widget.
