I am not sure if this has already been asked/discussed here but I couldn't find an answer to my question yet.
I would like to test, if it is possible to setup a form that needs to be filled out by a requester.
For example: If an employee wants to raise a ticket for giving an external access to our WiFi, I would like the requester to fill out a custom form that requires details from that guest that he needs to fill out before IT receives the request.
Does anybody know how this can be established? I could only add this form for the IT employee to fill it out when the ticket has already been created. In order to smoothen the workflow, I want the requester to fill it out. Any idea? I am happy to hear your thoughts.

Go to Project Settings > Forms. You can create a Form there.

I could create a form, that's not a problem. My question was, if this form can be filled out by the requester when opening a new request. Something like, when opening a request from type x, the requester has to fill out the form before submitting the request.
I could only manage to add the form after the request has been opened.

Oh, so they are creating the issue outside of using the portal and you want to force the user to use the portal view with the form. Correct?
Inside the form when editing, go to the Settings tab. Click on the option for Create new issue.

Exactly, If the user wants to open a new request, based on the selected request type, a form should appear that the user has to fill out.
Thanks for the hint. I also found that but when testing it, it didn't appear automatically. Probably will check it with a simple user as I am an admin. I will try it out and let you know. But thanks so far for helping me out!

Yes, I will be interested to know what you find out. When I test it here, it opens the form for me to fill out when I click the Create button.

I tried again this morning and I still don't see the form. Also after clicking on the create button nothing happens. I need to manually add the form afterwards.
Am I missing anything? This is what I see currently when opening a new request:

Hi John, I think i found the "issue".
I tried to create a ticket out of Jira itself which resulted in the form not being displayed. When opening it via the portal, like a user would do it, I found the requests with my custom forms. It is working now :) Thanks a lot for your help during that phase!
