hello, in the JSM tickets how and where do I create the fields at the top?

In addition to @Muhammad Moazzam Hassan, you can go to project settings -> screens -> screen configurations (select your screen) and click on 'Add Tab' to customize as you wish.

The fields can not be specific to the ticket but can be specific to Request Type and Issue Type, you can place the field on top but below the summary when the request type field is not empty, you can click on configure and drag the desired field up.
