Error when creating issue thru Gmail Extension / App

I installed the Atlassian G-suite app across our domain, it used to work and I believe since I did some change in permission it stopped working, I can't recall the change and there's no backup, so.. 

when I try to create an issue via Gmail, I can choose a project but can't create the issue, I get the screen below with the text "Something went wrong!"

I also included several roles for the 'create issue' permissions for that project, 

• Project role
• Users
• Developers
• Administrators
• Service Desk Team
• atlassian-addons-project-access
• Service project customer - portal access
  

it happens across all the projects

I am experiencing the same problem.  Have you been able to get resolve this issue or did you give up obtaining a resolution?
