(Sorry sooo many questions raised recently)
I need to design a form that has 3 drop boxes, within condition sections.
The contents of the 3 drop boxes are same. I don't mind linking to 3 separate custom fields, but is there any way for the custom drop boxes to use a single source for the list data?
This makes maintaining an update list more efficient.
Unfortunately, you cannot have multiple elements in the FORMs setup mapping to the same Jira field. Even though logically speaking you should be able to since it sounded that your setup is showing the different dropboxes under different conditions.

I'm concerned, by having multiple custom fields with their own list, it could come out of sync.

This is not possible as each custom field's options are associated to the its specific field. One thing you may want to consider if all three dropbox FORMs elements absolutely needed to be mapped to a Jira field. If not, then you can just manage the FORMs configuration to list out the options. User's input can just be captured in the FORM only and it is still associated to the issue.
