Don't see issue if i created in slack

I have service desk channel, and i set integration in slack but,

When i crated Issue in slack use /jira created [task name]

I don't see in queue my issue.

But on service desk portal it's all work

I find issue, they don't have Request Type. I checked Slack form, but don't see that field in template.  How i can edit template for slack?
