Does anyone know how to add a custom form when adding a transition in a workflow?

I am creating a onboarding process flow for my company. When the applicant moves, there are specific tasks, in which I created individual forms, to be completed by the person. So when I update the workflow and have it assigned to the specific person, I would like to add a custom form for the assigned person to complete. Can this be done?

I was also looking for issue view forms to guide agents through a workflow but have yet to find one a solution using forms or screens but we ended using checklists for our onboarding process.

As the onboarding record moves through the workflow different checklists are added via automation and in the workflow we added a condition that the record cannot move to the next status until the checklist is complete.
