We want to be able to set up SSO access for multiple users from multiple companies each to have access to their own service desks within Jira portal.

Does this mean we have to run our own ADFS server for all our customers, or how can we let them use their own MS login to join our service desk?
Do we somehow need to sync everyone in all external customers into our JIRA directory?
Can we have multiple external federated auth. providers?
Or how do we do it please?

Based on your query:
You don't need to run your own ADFS server for all customers. Instead, you can configure multiple IDPs to allow users to log in using their own Microsoft credentials.
You can connect multiple IDPs in your instance and not create their ADFS accounts. The customers will ultimately lead to your Jira directory itself.
