Custom forms and the Jira app

I created a custom form for maintenance requests and while it works great when viewing the ticket on a web browser,  through the jira app, it doesn't display the information,  only the ticket info.    What am I missing or is this a limitation of the platform?

You're not missing anything, Forms for Jira is currently not viewable through the mobile app. There's an issue out there to remedy that. Feel free to vote on it to signal to Atlassian that this feature is important.
