I have a form, created in a simple way, but I want to have conditional fields, for example, if a user chooses an option from the dropdown menu, it shows a text box and if not, it does not show. Is this possible to do?

You have to create a form and add a section to it. This section can be bound to single/multi select fields and not to text. Read more here:

Of course, it's possible! Forms (previously known as ProForma) are available in every pricing plan. Forms configuration is available under project settings in the service project. You have to add these fields to separate sections and define a condition when the next section is displayed, for example, when someone checked the ABC option in the previous section.

I am using a data center version of jira service management and the forms section doesn't come natively. Will the form section be available if i purchase a licence or will i have to download and pay pay for proforma even if i get a licence??

Unfortunately, in case of data center, Forms are an add-on for which you have to buy a separate license. What I know, there is no plans to make it as a native feature like in Cloud version.

You can create Jira dynamic forms using Smart Jira Forms add-on if you`re ok with Atlassian plug-ins. My team has developed this add-on to collect data, feedback and share form externally

You are able to create dynamic form using Extension for JSM plugin. It gives you more that this one possibility. You can also e.g. use Bundle Fields in your configuration (you can dynamically show more than one field, it is treated like one field from jira perspective).
