We are looking for an addon that can help us in creating advanced JSM forms with the following specifictions,
1) combining multiple forms in a single request
OR
2) Having multiple tabs in the JSM form
OR
3) Having a next button before submitting the form.

We are developing a dynamic Jira Service Management form to streamline request submissions through the customer portal. This initiative aims to enhance user experience and improve usability, specifically designed for non-licensed users to submit their requests efficiently.
At this point we have a lengthy form that displays all the fields which is not easy for users to work with.

For whom this app is intented to be used by? Customers or users? And kindly confirm that your need is something "Survey-ish". You want users to fill surveys using multiple pages.

Thsi app will be used by customers to submit the form. And form is not just a survey, it is a requirement submission form through jira service management.
