My Current Requirement is that when a User logs in the customer Portal user should get 2 Options in a drop-down and each of these individual drop-downs should contain 4 options. Now each of these 4 options would have their own multi-choice values in it along with a couple of description fields like Employee name, comments or attachments.

from what I have read here it seems that you would simply want to create a form with different sections based on the users input. Forms are pretty powerful.
Yes, I am looking for a form in such a way that I can configure multiple sub-form so that when I go in customer portal, I can see all listed forms in a browser to create an issue.

The way forms works is associated with a specific request type (RT) so when the customer goes to the portal they will have various RTs to choose from. Once they select a RT they will be presented with a form requiring them to input the requested info. A form can be structured in such a way that the requested info changes as the customer begins to input data.
