I am new to Jira. In my service project customer portal, I cannot see the form for the customers in order to submit a request ticket. I have checked that using a customer account as well as my project admin account. Cannot see anything using both accounts. I have given the proper permission and created the form for that portal.

Have you created Request Types for this JSM Project? This is what would display in the Service Portal

Hi @neranjan_v you need to have created a request type(s) and added the request type(s) to a portal group.
On the request types page near the top it states:
Make these request types available in your customer portal by editing your portal groups.
And on the portal groups page near the top it states:
Forms that aren't assigned to a group will be hidden from your customer portal.
