When you approve an issue, the status changes from a status 'Waiting for approval' to 'Approved'. You could add a post function to that 'Approve' transition that sends an e-mail to the person who is the Manager. If the Manager is the end user's manager and is a variable, then you need a custom field Manage (user picker) that can be used to send the e-mail notification to.
