My organization has built out about 75 different request types with set custom fields. We are in need of a quick fix with some conditional fields and would like to leverage forms without having to build a specific form for each request type. The information I am trying to collect is uniform and can be applied to all request types and must have all fields required. Is it possible for me to build a short form and place it above the static custom fields on every request type?

The short answer is no, you cannot add the form above the request form fields. The form will always be added to the end of the request type.
Thanks so much for your quick response! Would you have any other ideas based on your experience? We currently use a cascading field, but the drawback is that we need the second field that cascades to be required just like the first field. I appreciate it if you have any thoughts.

I would probably just remove the cascading field from the request form and then just add it in a Form and make it required. Then just add the form to each request type, I know it may not look the way you want it, but if the fields in the form are required the user would not be able to submit the request without filling them out.
