Automate adding of Jira Forms to a JSM ticket

Hi,

I have a "master ticket" that is created via portal request. This in turns create a bunch of tickets through automation, that is assigned to different teams.

In one of those tickets, an assignee (e.g. manager) gets a ticket for the tasks they have to do, including filling out a form (e.g. IT access provision), which will create another ticket for the IT team.

Creating the form by attaching it to a request type is not ideal, since this is an additional step, and I want this form to already pre-fill the fields that are available from the info provided before (from master ticket), which is done via linking the field to a Jira field. This info is helpful for IT team, who will be working on request and having the necessary information already on the ticket/form, and not having to check the master ticket every time.

However, I cannot find a way to automate the adding of the form. Assignee would have to manually add the form by following the steps below, which is a friction, specially for new users.

  1. Go to the issue you want to add a form to.

  2. Select the period key (.) on your keyboard, then find and select Add form.

  3. Find and select the form you want to add to the issue, then select Add.

This seems to have been possible before in ProForma.

Is there a way to accomplish this in Jira Forms and Jira Automation?

Above mentioned action (by @Antonio Rodriguez) does not seem to be available anymore. Instead there is a single option to "Copy ProForma Forms from Trigger Issue".automation_proforma.JPG

Is there any other way to attach available (template) forms to newly created tickets (not necessarily created from/having a trigger issue) by default? I.e. how do I set up a project automation to add forms to certain issue types, which are created directly in a project? 

@Adam Varadi I still see the same screen I posted. The UI looks different. I'm on Cloud, "new" experience as far as I know. Are we on different versions/UI?

@Antonio Rodriguez I forgot to mention that I am on Server (Jira v8.20.11).
