Attaching a Form Multiple Times on a Single Issue

I am managing a Jira project for Customer Support to submit new items to be created by Engineering. A single request is entered per customer Purchase Order. However, I would like to include a form to gather all of the relevant information for each item. There may be 1-10 different new item requests on a single Purchase Order and the form is meant for a single item.

A known solution would be changing the process from 1 request per Purchase Order to 1 request per new item. This is not ideal.

Here are a couple possibilities I have considered (unsure if either are possible):

1. Attach the item information form multiple times based on number input.

2. Create a form that copies sections based on number input.

It is my understanding that only a single form can be associated with one issue (i.e. Form submission to create the issue in JSM).

However, after the issue/request is created, it is possible to add new forms to the issue by agents - Please review the following videos from Atlassian

Another thing for you to consider is configure your single form to contain multiple section (using condition check), so based on a certain condition, then the section will be visible when users fill it out.  Afterward (based on your issue fields mappings), then you can establish automation rule to create subtasks or other new issues creation where you can issue link them back to the original issue.
