in request types > forms I can attach a form and it shows up in portal, but the attached part is missing when using Microsoft teams helpdesk > New request to create the same request
How an I get the attached form to be visible from anywhere someone raises that request?

The forms provided in JSM are currently only available in the application portal or issue view. They do not work in integrated applications currently.
