Attach a form to an email automation?

In JSM, if someone fills out a form and submits it through the portal, I want the info from that ticket to be sent to an email automatically. You can use smart values to include custom fields in the email but can you attach the form to the email?

Unfortunately, from what I know, and up to this date, this can't happen.
