Add form for external customers

Hello

I have a question regarding the creation of a form similar to Google Forms.

I need to creating a form that will allow customers outside of our organization to submit their answers. Additionally, it is important that we have the ability to download their submitted answers after completion.

if it's not passible how can i create external portal beside my internal portal

My team developed an add-on - Smart Jira Forms, which will allow you to share forms externally with users via link. Also the form can be shared as embed element on your website or Confluence page.

• no need to add a form to a Jira issue;
• one link - a lot of responses;
• link with no time limit for filling; sharing with users out of Jira;
• add a form link to HTML code of your website, blog, social media or Jira project to get responses from its visitors.

You can set up to create issue with form responses automatically and the form will be attached to issue. Alternatively you can export all responses in CSV. Hope this will help, let me know if you need further assistance!

Is it possible to modify the link to include my organization at the beginning of the link?

and Can I make changes to the appearance and layout of our form page?

Also, Is it possible to add the organization logo to our form page?
