I think you are saying that you have a checkbox field with five values or you have five checkbox fields. Either way you want to make sure these have a value before the the user can click create on the create screen.
You need to add a Validator to the Create action in the workflow. Depending on how you have the fields configured there are a couple that you can choose from.
Validates that one or more fields are not left empty during the transition.. I believe that one is out of the box. I have multiple workflow enhancing add-on's that I have used for years so I am hoping that it is.
