How to obtain additional values from customer portal

We are now using Typeform url to obtain additional information when customers click on the link to report their parking fine. The format is like https://niklas33.typeform.com/to/diHBye?puid=xxxxx&parkingid=xxxxx&start=xxxxx&stop=xxxxx&licenseplate=xxxxx&areano=xxxxx&popname=xxxxx&warningstatus=xxxxx

Now we are thinking about using the customer portal of JIRA instead.

So is there any way to fulfill the former task of obtaining additional data from customers?

The additional values are like parking starting time, parking ending time, area number, etc.
