Enhancement Request Process - Creating Issues in JPD via Form

I am working to create an intake flow for enhancement requests that come in to our Product team for discussion and prioritization.  Ideally, I am able to share a link to a customizable form where stakeholders can create an issue in JPD.  

Can I get advice on the best process for creating this form?
