Enhancement Request Process - Creating Issues in JPD via Form

I am working to create an intake flow for enhancement requests that come in to our Product team for discussion and prioritization.  Ideally, I am able to share a link to a customizable form where stakeholders can create an issue in JPD.  

Can I get advice on the best process for creating this form?

@Rohan Swami Hey! Has there been any updates in this area that could help our team out?   We are still in need of a way we can collect ideas & enhancements from people in our company that may or may not have access to JIRA. 

Ideal solution would be a form with a link that we can share with someone in our company, they fill out a customized form, and our team can control where this new request gets sent to, such as a dedicated JIRA Product Discovery Board to manage all of the requests.
