JSM Forms Automation

New to Jira and looking for some automation assistance.  In Jira Service Management (Cloud)

1 I have forms created and can add the forms in manually to Issues.

2 I created a custom field Default value= none Changed value=yes  where the selection of yes  will trigger automation to add a selected form to the issue.  

This is where I am running into issues. I am unable to figure out how to select Forms via automation. There doesnt seem to be a logical link to select forms.

Hi @Chad Jackson,

Welcome to Atlassian Community!

If you select the Attach forms action you have the option to select which form to attach, and if it should be internal or external.

Thank you,  I was able to get this working finally half the issue was our Insight fields were not populating correctly.
