Jira service management and Elements publish

I intend to use Elements Publish from the marketplace

I will be creating a custom form using JIRA service management

and will attach the form to a request type and customers can create requests using the form like 

I have the following questions:

1.  When a customer creates the request, can a new confluence page automatically be created using the Elements Publish app? If so how do we configure the automation? Exactly how do we do it?
2.  Once a request gets created can a customer come back and EDIT their old responses to the form which in turn edits the confluence page automatically?
3.  Will the form data be stored as fields and values in a confluence page? Can I access each form field and its associated value separately from within the confluence page if needed?

Since this related to a third party add-on and not a part of the JSM application itself, you should contact vendor's technical team for your asks directly - https://marketplace.atlassian.com/apps/1225519/elements-publish-create-confluence-pages-from-jira-issues?tab=support&hosting=cloud

We have not used this specific add-on in our envs.
