There are some good points here @Nazarii Sobchuk _Grandia Solutions_ 

We very rarely use a simple "What went well? What didn't go well? What should we change?" approach to our fortnightly sprint retros, instead I try and use targeted questions about different aspects of how we work and probably about once every 3 months the retro includes a question along the lines of "Do you know what our workflow is? Is it working? If not, what do we need to change?"
