why I can't choose the title for my cards when I am doing the intergration with Typeform?

I was stuck at the last step ' turn responses into cards'  when I was integrating with Typeform. I can't choose a title for my cards. there is nothing there to choose. 

Please help

I tested it. It seems that when my form is a single question, I do not have the option to choose. It could also be that my first and only question was a checkbox and it card name depends on my choice. When I add a second text question, I am able to now select a title for my card.
