card name automation

Hiya, does anyone know how I can automate card names using 4 custom field values? One of which is a date

I've tried loads of different formulas but none have worked

I can automate using a prefix-manually entered card title-{datenumber} but it will not let me select the value of of a custom field

Appreciate any help with this :)

Custom field variables have this format: {{%Field Name}}
