Why are duplicate checklists showing up on my cards in Trello?

I used my template checklist to create a card, but every item in the checklist shows up twice on the card? How do I correct that?

@Karen Kapler , could you provide a screenshot of what you're referring to?

I created a template with a checklist, created a card from the template, but saw now issues with my checklist. See below.
