Request for admin account access recover

My boss has lost access to his account. He doesn't remember the email or the password. when he tries to log in its sending a 6 widget Code to a email he doesn't have.   

Account info is for [NAME] [USERNAME] 

Other members are myself [NAME] [USERNAME] and [NAME] [USERNAME].

Our new address is [ADDRESS]  / Phone number [PHONE NUMBER].   

We kindly request guidance on verifying ownership and restoring admin access.
