We don't currently have an in-app document editor. When you download a file, any changes you make will not be automatically synced back to your Trello card.
In order to update the document on your card, you would need to delete the attachment, and then re-upload the document to that card. One option might be to use a third-party service, such as Google Docs, that supports real-time updating of your documents.
