We use Trello to record all our customer interactions and orders. Is there a power up/add on that we can use to record information in a document with bespoke text boxes that updates and tracks in real time? Any suggestions welcome.

To interact with documents, consider using our Google Drive power-up to attach files directly to cards.
For simple text entries, try using custom fields on cards, which allow you to add and monitor data in real-time. You can also setup automations that trigger based on changes in these custom fields.

Thanks for your comment. We are currently using a Word document that we use as a sort of questionnaire with our customers. We update the hardcopy then add it as an attachments to Trello. We would like to be able to access this (or something similar) from the Trello App and update it live within Trello. Any thoughts on what we could use please?

Editing Word documents isn't supported directly by Trello at the moment, so I won't be able to assist you with that.
However, I recommend exploring some third-party Power-Ups available in our Marketplace—you might find one that supports this feature! https://trello.com/power-ups
