I am trying to map my google form to a trello card. with specific title

 I need the title of the card to be like the first card, with a truck number, (my first question on the google form) and the second question which is issue  - instead im getting this response paragraph i don't want.   How can i map this correctly. ?

Yes form director,  I mapped it for NAME   to be my first and second answer - but only getting the response paragraph..    I need to add a rule too?

You can handle this entirely within Form Director. Change the Trello Name mapping type to Templated so you can combine your fields into a single custom title, like {Truck Number} - {Issue}.

Ensure the long paragraph is mapped strictly to the Description field, and test with a new form submission since old cards won't update retroactively.

Good Morning, 

     I mapped my google form, now when it is filled out and goes to my board, i get two cards, one like i want and one with the title "response to whatever i called the google form"  why would i be getting two responses for each google form sent?

If i do a new form, will the link i sent out still work or will there be a new link with the new form i make?

Sorry for the confusion!

You don't need a new form or new links just submit a new test response through your current form.

Receiving two cards means two integrations are running simultaneously (likely an older default setup alongside your new custom one). Open Form Director on your form, check for duplicate Trello services or active add-ons, and disable the extra one. If only one service is listed, reach out directly to Form Director support, as this is managed by their third-party integration and not Native Trello
