Download a Form

I have a Request form and I want to be able to add a form to download. The idea is a customer would download the form and fill out the details and then attach it back to the request form.

In the service desks request type you can add a link by using the following 

[link name|http://example.com]

This can be added to the portal instructions on a team managed project or in the request form help on a company managed project.  Both are found in project settings -> request type on the request type you would like to add the instructions to.
