I would suggest you read up on the overall JSM documentation, starting from this support article.
There is no such thing as "THE customer service desk webpage". JSM is a web application that has an overall umbrella page called the help center, and one or more service desks that are automatically added to that overall help center as you set them up. Each service desk is a JSM project with multiple forms called request types, which are all separate screens with their own URL.
