change create screen/ fileds based on reporter selection

Hi,

Iam using JSM data center edition, Is there any built-in or add-on that allow changing the displayed fields based on customer selection. 

EX: reporter is opening change and if he select open firewall port the other fileds changed to open box's for source ip address and destination ip address something like this

Jira Cloud has built-in functionality like Proforma (this plugin was bought by Atlassian and integrated to Cloud version). It's only for Service projects. Can be found here:

Also you can use Extension for Jira Service Management:
