change create screen/ fileds based on reporter selection

Hi,

Iam using JSM data center edition, Is there any built-in or add-on that allow changing the displayed fields based on customer selection. 

EX: reporter is opening change and if he select open firewall port the other fileds changed to open box's for source ip address and destination ip address something like this
