add more than one form to a service

I'm trying to add two forms on the service request page but only one appears, how should I proceed?

It is not clear on what is your asking exactly?  

If you are trying to associate multiple forms to an request type (i.e. so both forms show up as a part of the request form portal UI at issue creation), then it is not possible.  See existing enhancement ask tracked by Atlassian - https://jira.atlassian.com/browse/JSDCLOUD-4643

However, after the issue/request is created, it is possible to add new forms to the issue by agents - Please review the following videos from Atlassian
