Hi there,
I am exploring the possibility to create a two stage onboarding form. We currently use a single form that is filled in by HR. Once completed, it triggers the creation of several tickets related to the onboarding process.
I am wondering if it would be possible to create a sort of two-stage form that accomplishes the following:
- HR fills in first part of the form including name of the new employee's future manager.
- After HR fills in its data, the manager is notified and completes the remaining form (or second stage of the form).
- Once both steps are completed corresponding tickets are created (think prepare laptop, phone, workspace).
Is this something I could realise within Jira Service Management?
Hi Rik,
You need to create a user field named as Manager in your first form. then create an automation to notify the manager to fill the next forms.

Hey Geethanjali,
Thanks for your reply. Is this something I couldn realise in a single form or would two be required?
Kind regards,
Rik

Hi Rik,
Assign a different request and different issue type to the form to be filled(created) by HR.
After creating issue you need to create automation to notify the manager.
In that you have to do three levels.
level 1:HR form ->level 2:Manager form->level 3:(think prepare laptop, phone, workspace).
Create another automation to create level three tickets based on any field in the issue in the manager form.
let me know if you encounter any issues or have any questions.
Regards,
Geethanjali
