Status Not Changed post Transition "Approve" is Clicked

I have used a Workflow and have a request which start with "Pending Approval" status. I have transitions "Approve" or "Decline". If I chose to Approve the status remains "Pending Approval" post "Approve" transition click as well? Would there be any reason for the same?

@Rangarajan Parthasarathy, would you mind posting a snapshot of your actual workflow including the transitions? You will need to go to project > settings > workflow, edit the workflow and click the radio button to include transitions.

Thanks. So which approval is echoing the issue. You have three approvals. Are all exhibiting the issue?

CAB Approval is the one that is having issue.

so what i don't yet understand is you indicate you have a "status" called Pending Approval yet I do not see that here in your image.
