Do Service Desk Customer Portal Users (i.e. customers) require a paid licence? or do they not count towards licence usage?

Yes. Each customer requires a licence.

Atlassian are looking at some form of system that is more like "agent based" licencing at the moment, but I don't know any details or schedules for it yet.
