Run automation based on time in form field

Hello! I'm looking to run an automation based on a time set in a form.  For example HR fills out the form when the user is leaving and includes the date and time when this happens. I want the automation to start at this time indicated in the form. My thought was to use a scheduled trigger running every 15 minutes comparing the time in the field to now but I'm unsure of how to write the JQL for this. Or if anyone has any other ideas that would be super helpful thanks!

Can you confirm if your org is using FORM to trigger an JSM issue creation?  If so, it is not possible to change the issue creation date with the date that indicated/captured in your FORM by default.
