I would like to a add a Reject Screen when the approval status is decline on Operations Approval and Financial Approval.

Here is my current workflow.
Please let me know if this configuration is possible.
In your transition to Decline Request where it says Declined Approval you can present a screen, Screenshot below is using the "new editor" but once you click on the transition you can add it under "request input" like below, my example is for a resolution rather than a decline reason.

yes, this is absolutely possible, and Christopher is pointing you in the right direction 👍
In Jira Service Management Cloud, you don’t add a “Reject screen” to the approval itself. You attach a screen to the Decline transition in the workflow.
What you need to do:
Open Project settings → Workflows
Edit the workflow
Click the transition that moves the issue to Declined
In the new workflow editor, use “Request input”
Add the fields you want to show (for example: Decline reason, Comment, Resolution, etc.)
That screen will then appear when someone triggers the Decline transition.
One important detail: If you are using the built-in Approval feature (the Approve / Decline buttons in JSM), those buttons execute a workflow transition behind the scenes. So the screen must be attached to that specific transition that is linked to the “Decline” action.
If the screen doesn’t appear, it usually means:
The transition you edited is not the one actually triggered by the approval
Or the workflow wasn’t published after changes
So yes, configuration is possible.
You just need to attach the input screen to the correct Decline transition in the workflow editor.
