ProForma and Jira Insight

Is there any possible to use in jira proforma with insight fields?

We have moved some data into insight and now we are looking to use proforma app to make better end user portal.

Currently that is not possible, but it will be soon. Forms will have access to Insight fields sometime in Q3-Q3 2022 based on Atlassian's roadmap.

I notice this is for Cloud but DC?

I understand that Atlassian is Cloud first but DC users like also needed functional.

For DC it is still being considered, there are a couple of suggestions logged for this, FORMDC-27 and CONFSERVER-69272 are two that I found.

Without using Proforma Forms - 

You can create Custom field in your Jira/JSM env that is sourced from Insight.  Afterward you can add the custom field to your Jira/JSM project's issue screens configurations, then you can expose it to your Request Type Form (this is not FORMS) screen for your portal users.
